Cookie Dough Packets will go home Wednesday and above is the brochure informing parents and students how to earn different levels of prizes. Not shown: Sell 75 items for 4 Tickets to your choice of either Dallas Mavericks, Houston Rockets, Oklahoma City Thunder, or San Antonio Spurs.  Sell 100 items for all of the above plus $100 American Express Gift card. Sell 150 items for all of the items in Levels A-L PLUS an iPad-1!  Inside the packet is the cookie dough brochure and a money envelope.  ALL ITEMS ARE PREPAID and orders will NOT be honored if payment does not equal the amount shown on the order form.

Checks are made payable to White Oak Intermediate School and must include Driver’s License and Birth dates. Parents need to total both the order form and payments. Double check to make sure the total amount enclosed in the envelope EQUALS the total amounts of both order forms.
